3 EASY STEPS FOR 2021 ENROLLMENT


Open Enrollment will be held from November 4, 2020 – November 20, 2020  for an effective date of January 1, 2021. (401k Open Enrollment will be held from December 1 – December 15, 2020)
All forms due on or before 11/20/2020 to your local office or HR.

Medical Plan


Q: If I want to keep the medical coverage offered by this plan is paperwork needed from me?

A: No, paperwork is not needed. Unless you qualify for the “non-tobacco credit,” you must complete the  Non -Smoker/Tobacco Credit Form to be entitled to the weekly $5 credit.

Q: If my medical coverage has any changes, additions or deletions is paperwork needed from me?

A: YES, Complete the CRB Medical & Dental Enrollment Form New ID cards will be mailed.

Dental Plan


Q: If my dental coverage remains the same as the prior year is paperwork needed from me?

A: NO paperwork is needed.

Q: If my dental coverage has any changes, additions or deletions is paperwork needed from me?

A: YES, Complete the CRB Medical & Dental Enrollment Form Under this plan you have the choice of using the Aetna Dental Network provider to obtain discounted dental rates. Click Here to find a participating provider in your area.

401k Plan


If you are enrolled in the 401k plan and would like to make changes, please log-in to the Empower website after December 1, 2020. If you are newly eligible to the 401k plan and interested in enrolling, Empower Financial will send a packet with your PIN number.

Q: If I am currently participating in the 401(k) plan and do not want to make any changes is there paperwork needed from me?

A: No, paperwork is not needed.

Q: If I am currently participating in the 401(k) plan and want to change the weekly % I contribute is there paperwork needed from me?

A: YES, Complete the 401k Contribution Change Form

Q: I am newly eligible (or had prior eligibility), and would now like to participate in the 401(k) plan. Is there paperwork I need to complete?

A: YES, Complete the Empower 401(k) Enrollment Form and the 401(k) Beneficiary Form.
